Ketamine-Assisted Therapy (KAT)

Ketamine-assisted therapy can offer sustainable relief for individuals struggling with depression, anxiety, PTSD, OCD, social anxiety and trauma, including a reduction in symptoms previously unresponsive to SSRIs or traditional talk therapy. On many occasions, we see individuals that have previously tried ketamine therapy (administered in medical environment through an IV infusion or intramuscular injection) who have not experienced sustainable results, who have plateaued or experienced extreme discomfort with injection-only sessions, and/or who prefer a more supportive and comfortable setting.

When administered in a safe, supportive environment with a trained therapist, ketamine acts as an accelerant and a medical ally, promoting deeper understanding, psychological flexibility and new perspectives. It is during this 24-48 hour window of time —called the neuroplastic window— that working with a therapist trained in integration is extremely helpful in not only navigating your experience, but in making sense of it.

At EXPANDED, the integration goes beyond light discussion. Gianna, your guide, is a Board Certified Behavior Analyst with over 20 years of experience. This means that, in addition to her training in ketamine-assisted therapy and psychedelic harm reduction and integration (PHRI), her primary expertise lies in helping individuals turn insights and illuminations (which are sometimes unclear or esoteric) into clear, actionable and sustainable changes.

NeuroSoma room access is complimentary for ketamine participants, and is an add-on support for other services. The space includes a BioMol red light for cellular respiration and joint health, Higher Dose heated PEMF mat, 40hz Gamma light, 4-8hz theta wave playlists, organic snacks and beverages, and more.

We understand that talk therapy doesn’t work for everyone, and that intellectualizing anxiety, depression or trauma may at some point result in a plateau, or even create an unnecessary focus on the symptoms you are trying to alleviate by reinforcing neural pathways associated with limiting beliefs and harmful stories.

In Theta Wave sessions, Gianna leads bespoke, trauma-informed experiences that alchemize and reconnect the brain and body, which use theta wave (deeply relaxed) states to metabolize, process and rewire the brain around specific events or general emotions or feelings of being “stuck”.

These sessions can be deeply fulfilling, and most members find relief and clarity within 1-2 sessions.

Members are given an EXPANDED Method notebook as a take-home tool to be more present in the process, track insights and take space for integrating insights into daily life.

Private and Small Group Sessions

EXPANDED now offers one-of-a-kind experiences rooted in Gianna’s unique training on the East and West Coasts. Bridging science with spiritual, each private or semi-private (capped at 4-6) class is designed for individuals who want an additive and expansive “third space” — leaving you feeling restored, uplifted, educated and reconnected to your mind and body.

Psychiatric consultations can be cost-prohibitive, ranging between $500-$1,000 for an initial session in the DC Metro area. EXPANDED runs differently. In order to remove a common cost barrier to ketamine access, and to use your time investment wisely, we have built a rare and unique comprehensive assessment:

  • Detailed psychiatric evaluation, evaluating mental, emotional, and physical health factors contributing to psychiatric symptoms

  • Diagnosis of mental health conditions, if applicable

  • Assesses cognitive, emotional, and behavioral symptoms

  • Discuss nutritional deficiencies, inflammation, hormonal imbalances, and gut-brain

Functional Nutritional Psychiatry

According to the Institute of Functional Medicine, the emerging field of nutritional psychiatry acknowledges a positive relationship between diet quality, mood and mental health. Recent research suggests that while nutritional habits, dietary patterns, and diet quality may all effect overall mental wellness, they may also be preventable or modifiable risk factors for mental disorders.

Your functional psychiatry sessions are tailored to your specific needs and may include:

  • Identify and address micronutrient deficiencies that impact mood and cognition (e.g., Vitamin D, B12, Omega-3s, magnesium, zinc)

  • Provides guidance on amino acid therapy for neurotransmitter support (e.g., 5-HTP for serotonin, L-tyrosine for dopamine)

  • Includes Integrative Pharmacology & Supplementation

Sleep + Circadian Rhythm Enhancement

Circadian rhythms are closely linked to mental health, and managing them can help improve mental health outcomes. Disrupted circadian rhythms can contribute to mental disorders like anxiety, depression, and psychosis, and can also be an early sign of relapse for mental disorders. Psychiatric symptoms may also worsen with poor sleep.

Similar to our functional nutritional psychiatry sessions, each meeting is uniquely designed for your current needs and experiences. Your session may involve:

  • Assessing and manages insomnia, circadian rhythm disruptions, and sleep disorders secondary to psychiatric conditions

  • Learning and practicing cognitive-behavioral strategies for insomnia (CBT-I)

  • Prescription of non-controlled sleep medications as appropriate

  • EXPANDED is considered “out of network” with all insurance providers. However, you will receive superbills to submit to your insurance provider for all eligible visits. We cannot guarantee reimbursement for all services, as this decision is determined by your insurance carrier.

  • Absolutely. EXPANDED warmly welcomes all individuals assigned female at birth (AFAB), including transgender, nonbinary, and gender non-conforming people. Our practice is designed to be a safe, affirming space that honors your identity while addressing your specific health needs and experiences.

  • Yes. We welcome men who are seeking our services and who respect our female-focused environment. We're committed to providing excellent care to all of our community members

  • We offer both in-person appointments at our Washington, DC office and convenient telehealth services via Simple Practice, a secure HIPAA-compliant platform. We currently serve patients throughout DC, Maryland, and Virginia. Virtual sessions are held under specific circumstances only, so please book a consult if this is preferable for you.

  • Yes - while under medical supervision by a provider/doctor or nurse practitioner, and while working with a trained therapist, ketamine is not only extremely safe, but has been shown to positively address social anxiety, depression and other mental health challenges. Gianna is passionate about leveraging her education at Stanford University’s Graduate School of Business, where she studied Social Entrepreneurship. She dreamt of a business model that honored the spirit of this work by positively contributing to the collective. It made sense to build EXPANDED using a matriarchal, regenerative business model, so that individuals investing in their own healing and betterment would naturally contribute to someone in need.

A percentage of net profits are reinvested in pro bono or reduced cost therapy sessions to support individual, family and community mental health.
